Z133 expects a job briefing before work begins and again whenever conditions change. This form makes it quick: write the 911 address and nearest cross street, the day's tasks, who's doing what, and check off the hazards on site — electrical, traffic, drop zone, weather, dead or hung limbs.

It captures the nearest hospital and muster point, runs an equipment and PPE check, and ends with a crew sign-off so everyone confirms they heard the plan. Documented, dated, defensible.
